Information
Requirements needed to apply for U.S. Passport / Passport Card /
10 YEAR PASSPORT / PASSPORT CARD - ISSUED FOR
16 YEARS OF
AGE AND OLDER:
The passport card can only be used for land and sea
travel between the United States, Canada, Mexico, Bermuda and
the Caribbean.
Instruction

- Certified copy of your birth certificate with raised
seal or original naturalization papers, or an expired U.S.
Passport.
- A completed and unsigned application.
- Passport photo.
- Valid Driver's License or State ID.
- 2 Payments - total for a ten year passport $165.00.

1st payment made payable to the "U.S. Department of State" in the amount
of $130.00.
The only accepted forms of payment are Checks ( personal, certified,
cashiers, travelers),
Money Orders (U.S. postal, international, currency exchange), and Bank
Drafts.

2nd payment to "Candice Adams, Circuit Court Clerk" in the amount of
$35.00.
The clerk will accept cash, check and credit cards.
If you pay by credit card, the clerk shall collect a service fee of
$4.00 for any payment up to $200.00.
If the amount due is $200.00 or greater an additional service fee of 2%
will be collected. [705 ILCS 105 / 27.3b]
Passport cards are available for an additional $30.00 fee
payable to the U. S. Department of State.
The clerk will accept cash, check and credit cards. If you
pay by credit card, the clerk shall collect a service fee of
$4.00 for any payment up to
$200.00. If the amount due is $200.00 or greater an additional
service fee of 2% will be collected. [705 ILCS 105 / 27.3b]
5 YEAR PASSPORT / PASSPORT CARD FOR A MINOR (UNDER 16)
All children must be present in front of an acceptance
agent if applying for a U.S Passport.

- Certified copy of your birth certificate with raised seal
(long form with parents listed). If child has had a previous
passport a long form birth certificate is still required
- Passport photo
- Both parents must consent to any child under the age of 16
Parents can do this by appearing in person at an acceptance
agency with a valid Driver's License or State ID. If one parent
cannot appear a form DS-3053 "Statement of consent for non
appearing parent" can be downloaded from web site
http://www.travel.state.gov This form MUST
be notarized.
- 2 Payments - total for a five year passport $135.00.

1st payment made payable to the "U.S. Department of State" in the amount
of $100.00.

If you are traveling in less
than 14 days you will need to make an appointment at the Chicago
Passport Office at (877)487-2778.
RENEW PASSPORT / PASSPORT CARD BY MAIL
To renew a passport / passport card by mail the applicant
needs to answer yes to these 4 questions',
- I can submit my most recent U.S. passport
- I was a least 16 years old when my most recent U.S.
passport was issued
- I was issued my most recent U.S. Passport less than 15
years ago
- I use the same name as on my recent U.S. Passport; OR, I
have had my name changed by marriage or court order and can
submit proper documentation to reflect that change.

This procedure can be done by applicant without having to

the U.S. Department of State
